If the pulse width & duty cycle gauges are reading full, does that mean that the injectors are at their maximum flow rate?

For pulse width the answer is no, but for duty cycle the answer is yes. (When you are at 1000 RPM, say, the window of opportunity is 120 ms, so running with a 25.5 ms pulse only means they are using about 20% of their capacity.)

I've seen situations where the commanded duty cycle exceeds 100%, often it's the case that the engine is tuned way too rich and the injectors are still big enough. On the other hand, it could be that the injectors are indeed too small, you need to also look at the AFR and see if the thing is rich or lean.
