I hope you realize that the stock KTM coil is a cdi unit. You need a coil for an inductive ignition system, about 3 ohm primary resistance.

Just make a short spark plug wire that goes from the COP to a stock spark plug boot. Also I have seen some timing lights that would trigger off of the coil primary wire using the inductive pickup.
You definitely need to set the timing with a light.

You can T the vac off of all four intake tracts. Some people have had luck with about a .030 restructure in the vac line.A mig welding tip works good. There is a tradeoff between response time and a steady vac signal.You might have to play with the size to achieve this.

What we want to see is a wiring diagram of how you physically have your coils wired from the Microsquirt,to the 211 and to the coils. Also, do you have good information on what the dwell setting should be for the coils that you are using. If these are motorcycle COP's make sure they are not cdi ones ...

Just a few thing jump out.

In injector settings you have your # of cylinders and injectors set to 8, should be 4, that is why it is firing every 90 deg. Looks like your cranking rpm is a little high.
In other fuel settings you have tps flood clear set to 1%, should be 70%. You have AFR Stoich set ...

Looked at your msq. You have your offset for output 2 set to 0, it should be 180.
Time mask should be 0.2 and percent 10.
Are you sure your cam sync is triggering on the missing tooth?
Have you tried it with no cam sync, just single crank wheel wasted spark?
I don't know why you are running cam sync ...

You are probably not going to get much help unless you post a msq and a data log.
But right off the bat, you have to set the skip teeth to six six or it is only going to fire once per revolution.
Have you checked tour timing with a timing light?
What tooth is your crank sensor on at TDC?
Where is ...
